If you need to update an individual’s details on Medicare, please use this form Appendix B- Indigenous Medicare appllication form
If you get an error from the Assisted registration process which states something about an IHI not being identifiable, then chances are that either:
1) you have entered the Name, DOB, or Medicare card number wrong
or
2) the details DHS holds on your patient do not match what is on the form (in which case, updating these details using Appendix B- Indigenous Medicare appllication form may help).
